Center for Geriatric Health Coordinated Services
To ensure continuous patient satisfaction, Olympia Medical Center provides ongoing care coordination with the patient, family members and personal physician. This means that whether your older adult is in need of a cardiologist, a foot doctor, or needs a hearing test, it can all be arranged through the Center for Geriatric Health. We will also coordinate care with the home health and community agencies.
Following your appointment, a nurse or social worker will follow up on your progress and coordinate care with family members and your physician.
