Physician’s referral
You will need a phyician referral for outpatient rehabilitation services. We work with most insurance providers. Plus, we can arrange for special appointments (see below) and transportation.

Hours of Operation:
Outpatient: 7:00 am – 5:00 pm, Monday – Friday.
